Delivers a variety of support services aimed at individuals facing intellectual and/or developmental disabilities. These services may encompass case management, tailored support plans for individuals and families, access to brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational training.
Intake Procedure: Contact to obtain an application or arrange an intake appointment.
Travel/Location: Take Exit 304 off of I-84.
Fees: No charges apply.
Who can use this
- Individuals with an intellectual or developmental disability who are residents of Oregon and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
