About this resource
Delivers a variety of support services aimed at individuals facing intellectual and/or developmental disabilities. These services may encompass case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational support.
Intake Procedure: To initiate the process, call to obtain an application for Developmental Disabilities services. An eligibility worker will schedule an appointment to guide you through the application process.
Fees: There are no charges.
Who can use this
- Applicants must have an intellectual or developmental disability, be residents of Oregon, and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability
